The following are examples of Lotus Forms integrated into a Domino Environment.


The environment used includes:
IBM X-Series Server running Microsoft Windows 2003 Standard Server
IBM Lotus Domino Version 7.0.1 - Domino is providing the Authentication & Security, Database Store for the Lotus Forms Templates, Database Store for prepopulation data, and Database Store for extracted data.
ISW Workflow-Xpress Version 2007.2 - Workflow-Xpress provides the workflow and business process requirements for each of the forms.
IBM Websphere Application Server Version 6 - WAS acts as the servlet container for the Lotus Forms translator servlet (Translates Forms into HTML) as well as the servlets required for Domino Integration

Forms Gateway demo (Demos a number of forms including the ones listed below. Pixel Perfect, Click Wrap, HTML Rendering, Database integration, Domino Integration)
In this demo we see a prototype Lotus Forms Gateway for an Australian Utility company. The concept is one of a Forms gateway that a user would log onto to see a list of available forms. The user can open the forms and submit them either as drafts (which are then accessible via in-progress views for completion) or as completed. This demo could be enhanced to demonstrate forms that are submitted into a workflow process using Workflow-Xpress integration with Lotus Forms.

TPTL Referral Form Demo (Demos Pixel Perfect Forms, Click Wrap Signatures, Rendering as HTML or in Lotus Forms Viewer)
In this demo we see a form that is historically a paper based form. The form has been developed using Lotus Forms Version 3.0 and has been designed to be a pixel perfect version of the paper based form. As you can see from the demo both the HTML version and the Viewer versions (Which are actually the exact same Lotus Form that is just rendered as HTML or in the Viewer) as almost identical. This capability of Lotus Forms to have pixel perfect forms is critical in environments that have compliance requirements for forms to look exactly as their paper version do.

This demo also includes an example of the Click Wrap Signature method. This allows a form to be electronically signed and wrapped up so that it's integrity can be maintained. Once signed the form can no longer be modified and any tampering of the XFDL file will result in the form being invalidated.

The TPTL Referral Form template is stored in a Lotus Notes database.

Lotus Forms Mortgage Application Demo (Demos Form Wizards, Complex Calculations, Prepopulation of data into form, Click Wrap Signatures)
In this demo application we use the Form Wizard capabilities of Lotus Forms to improve the user experience. The Forms Wizard allows us to break the form into logical sections The demo also shows how Lotus Forms can perform complex dynamic calculations etc as part of the user filling in the form.

The Mortgage Application Form template is stored in a Lotus Notes database which also contains customer records. When the customer enters their Customer No the form is prepopulated with their personal information so they don't have to re-enter the details. Once the form is completed and submitted a new document/record is created in the Lotus Notes database that attaches the Mortgage Application. In addition key information is then extracted from the XFDL file and populated into the Lotus Notes document so users can see the information without opening to form.
